child enrollment step by step process

Enrollment process

1. When determined the need for childcare, please check with Milwaukee Enrollment Services for childcare eligibility and authorization at 1(888) 947-6583 if using subsidy payments from the state of Wisconsin.

2. Take A Tour at Shiggy's Playhouse! Look Around, Ask Questions, and Determine if this will be the right childcare home for your family. 

3. Read over Shiggy's Playhouse Program Policy  to gain a mutual understanding and expectation from both families and the provider. Key reminders and facts will be gone over in the Parent/Provider Contract.

4. Once decided that your family will choose Shiggy's Playhouse for your childcare home........Express Interest to Save An Enrollment Spot if Needed. Contact: Ms. Cherrie (414) 309-3045 Spots are saved for 3 days, after 1 week at $100 payment must be paid in order to continue saving the enrollment spot/s.

5. Determine 1st date of attendance, communicate that intention to the provider for mutual agreement.

6. If using subsidy, use Provider #0000588370-002 to sign your child/ren up for our childcare center.

7. Complete The Childcare Enrollment Packet As Soon As Possible!

The following forms must be Fully Complete (no missing spaces, put a line for information that does not apply) and turned in BEFORE or ON the First Day of Attendance.

A. Enrollment Form: (Click HERE for digital version to print)

B. Signed Program Policy Contract and Parent/Provider Contract

C. Health History & Emergency Care Plan: (Click HERE for digital version to print)

---The following forms must be completed and turned in within SEVEN business days of the child/ren's first day of attendance.---

D. Immunization Record: Use your child's Social Security Number, Medicaid ID Number or Health Care Member ID Number to lookup and print your child/ren's public immunization records HERE or obtain from your child's doctors' office. Click HERE for the digital version to either give to your doctor or sign-off of refusal for religious reasons or personal conviction/s.

E. Child Health Report: Needed for children under the age of 5. Click HERE for digital version to print.

F. Alternate Arrival: Pick-up & Drop-off  (Click HERE for digital version to print) The Provider must know the schedule of when to expect the child/ren for pick-up and drop-off. 

8. Make Childcare Tuition & Registration Fee Payment. If using WI subsidy, payments are made through the parent (cardholder) portal of ebtedge.com or by calling 1(877)-201-7601 using your ebtedge card. All WI subsidy payments are due by the 2nd of each month. 

9. First day of attendance!

A. Bring parent provided belongings.

C. Express any other questions/concerns or information about your child/ren.
